General information from the Pea team. Not medical advice or a clinician-reviewed treatment plan. How we write.

In this guide

Use this free planner to organise what you want to discuss at an appointment. You do not need a complete diary or perfect wording. A short note about what has changed and what matters most to you is a useful starting point.

What to bring

Questions you can adapt

“Could anything else explain these symptoms?” “What are the options for my main concern?” “What follow-up should I expect?” “When should I seek help sooner?” Choose the questions that fit your situation rather than using this as a required script.

Keep facts and guesses separate

“I noticed this on these dates” is different from “this caused it.” You can share a possible pattern while making clear what you know and what you are wondering. NHS guidance notes that symptoms associated with PCOS/PMOS can have other causes, which is one reason professional assessment matters.

Privacy and saving your note

This tool has no account and does not upload your entries. They are not saved if you refresh or close the page. Use Print to keep a paper copy or save a PDF on your own device. Be mindful of shared printers and devices when saving health information.

For ongoing records, Pea can create a visit summary from information logged in the iPhone app. This free planner is separate and does not import your Pea records.

This is preparation for a conversation, not a diagnostic tool. If you need urgent care, use your local healthcare or emergency service rather than waiting to complete a checklist.
